Trans Elder Health Issues

Transgender elders face the same major health concerns as other elders - heart disease, cancer, stroke, etc. - plus some. This article examines systemic barriers transgender elders face, common health problems with which transgender people experience disparities, and health problems that are specific to transgender people.

Transforming Mental Health Services for Older People: Lesbian, Gay, Bisexual and Transgender (LGBT) Challenges and Opportunities Loree Cook-Daniels   7/25/07

A paper prepared for the
AARP Diversity and Aging in the 21 st Century Conference
June 21, 2007
Los Angeles, California

Service providers often think about “minority” group members as challenges: How do we adapt our existing protocols and programs to meet the needs of these people over here who are different from our usual clients? Unfortunately, we frequently attempt to answer this question by trying to gain “cultural competency,” a code phrase that all too often stands for this directive: “substitute this (just slightly more nuanced) set of stereotypes for the set you have been using.” This approach neither serves individual clients – who can never be summed up by a single attribute, whether that be their race, age, gender, sexual orientation, or psychiatric diagnosis – nor makes full use of the opportunities for systems improvement that diversity creates.

This article offers alternatives to this standard (and inadequate) response to diversity by exploring four ways in which engaging aging diversity can actually improve both our services and the broader community.

Femmes, Butches and Lesbian-Feminists Discussing FTMs
Feminists, particularly lesbian feminists, have expressed many concerns about the decisions and "meaning" of FTMs. What happens if we turn those same concerns and questions upon ourselves and our identity? What can each community begin to learn?

"Serving Trans Elders: What Service Providers Need (and Don't Need!) to Know"
SAGE Conference Overheads

On June 18, 2004, Transgender Aging Network (TAN) Director Loree Cook-Daniels presented a workshop on "Serving Trans Elders: What Service Providers Need (And Don't Need!) to Know" at the "No Need to Fear?  No Need to Hide?  Aging on Our Own Terms" conference on Lesbian, Gay, Bisexual and Transgender aging in New York City.

The following diagrams were used in this workshop to tease apart the primary features of being transgender that might, depending on the service being provided, be relevant for service providers to know.  At the request of participants, they are being offered here.

Making Space Safe

In this 2001 article reprint (from the True Spirit Conference Program Book), Cook-Daniels examines just what inclusion means and how space can be "made safe". This article breaks free from old concepts of "safe space" and offers many useful alternatives for organizers, leaders and others to use as guidelines.

Transgender Elders and SOFFAs: A Primer for Service Providers and Advocates

This paper is one of very few articles specifically focused on aging trans and SOFFA (significant others, friends, family and allies) issues. The paper covers definitions, the social implications of transition, issues specific to transitioning in later life, the role mental and physical health professionals play in transition, SOFFA issues (focusing specifically on spouses and partners, parents, children, friends, co-workers, neighbors and clerks), health care issues in later life, legal and financial issues in later life, and social concerns. The final section discusses implications for service providers and advocates, including a discussion of the challenges involved in integrating heterosexual (self-identified and/or perceived) trans people and SOFFAs into groups traditionally focused on lesbians and gay men.

Lesbian, Gay Male, Bisexual and Transgendered Elders: Elder Abuse and Neglect Issues

Still the only article published in the professional literature devoted to LGBT elder abuse issue, this article (which originally appeared in the Journal of Elder Abuse and Neglect (The Haworth Maltreatment & Trauma Press, Vol. 9, No. 2, 1997), briefly defines the social, psychological, and legal environments of lesbian and gay elders, and discusses in greater depth the range of abuse specifically targeted at lesbians and gay men, including focus on topics such as internalized homophobia, legal barriers, and fear of authorities. A separate section on transgender elders covers definitions, non-congruent bodies, and transphobia. The paper closes with practical suggestions on how to serve LGBT elders successfully and respectfully.
